I’m writing to inquire about the process for changing my last name with NSBEP. I am re-assuming my maiden and am currently registered under my married name. Could you please advise on the process to change my name with NSBEP? I am able to provide any documentation (e.g., driver’s license, divorce certificate) that you may require.

A Driver’s License would be fine. For future, here is the policy (page 4 under change of name heading):

Once you receive this information from her then you can change her name in the database (and QuickBooks).
