Is documenting that in their file enough or would it be better to have clients fill out a form and sign that I include in the file?Ok great RE: address – I’ll be using my home address for everything (the Board, CRA, business bank account, registration of business, etc.), except on documents/correspondence that clients have access to (letterhead, website, business cards etc.) would be my PO box address, but the physical location is my home and where everything will be stored so sounds like I’m in line with that directive.

One other question I had was that although I’ll be the custodian of the file, I will need to pick someone who could take over in the event of my incapacitation in some way (illness, accident, death) correct? Are there guidelines on who that should be or can it be any psychologist named?
